Frame-2

Prepare Your Imaging

Prepare the imaging related to the condition you are being seen for

• MRI, CT scan, X-ray, or other scans — ideally completed within the past 6 to 12 months.
• Bring both the imaging disc or secure copy and the written image/radiology report.
• You may also send these ahead of your visit to Email: records@denovobrainspine.com

Important: If you have MRI, CT, X-ray, or other imaging studies, we recommend requesting a copy of the images on a disc and bringing it with you to your appointment. While imaging centers may offer to send them directly to us, bringing the disc is the most reliable way to ensure Dr. Shannon has access to your imaging during your visit.

Frame-3

Send Records Ahead Of Time

Send outside medical records before your visit

Dr. Shannon reviews your imaging and medical records directly with you as part of your visit. Receiving these materials in advance helps him prepare for your clinical evaluation and supports a more complete review of your condition.

• If another provider has records related to your condition, please have those records faxed to our office before your appointment.
• Request for your provider toFax records to: (404) 891-9299
• Include your full name, date of birth, and appointment date so we can match your records to your file.
